Nursing care and emotional support for those in the last six months of life, in patient’s home or in a facility. Primary focus is patient- and family-centered care, pain and symptom management. Medicare, TennCare, private insurance.

In-Home Hospice

In-home hospice allows terminally ill patients to spend their last days in familiar surroundings. If the patient is enrolled in Medicare, choose a Medicare-approved hospice provider.

Facility-Based Hospice

Hospice is available to nursing home residents and hospital patients. Some nursing homes offer hospice by contract with a home health care agency, or the resident or family may contact an agency themselves. Ask nursing home staff about arrangements. Hospice patients who meet criteria can be admitted to a hospital transitional care unit on doctor’s orders, if care of a dying patient is more than family can manage.
