Description:
Tennessee’s CHOICES program provides long-term services and supports for older adults (age 65+) and adults with physical disabilities (age 21+) who meet eligibility requirements. Services may be provided at home, in the community, or in a nursing facility. CHOICES offers Home and Community Based Services (HCBS) to help individuals with daily living activities, remain in their own homes, stay active in the community, and maintain employment when possible. When needed, CHOICES can also provide care in a nursing facility as an alternative to HCBS. Proud sponsor of the 2025-2027 Aging Well Resource Directory.
