Hospice
Nursing care and emotional support for those in the last six months of life, in the patient’s home or in a facility. Primary focus is patient- and family-centered care, with attention to pain and symptom management. Medicare, TennCare, private insurance.
In-Home Hospice
Many terminally ill patients prefer to spend their last days at home, surrounded by familiar people and things. Many home health care agencies offer in-home hospice services. Call your choice of home health care agency to find out if it has a hospice program.
Facility-Based Hospice
Hospice is also available to nursing home residents and hospital patients. Some nursing homes offer hospice by contract with a home health care agency; in others, the resident or family members may contact a home health care agency themselves to establish the service. Ask the nursing home staff about those arrangements.
Hospice patients who meet admission criteria can be admitted to a hospital transitional care unit, on doctor’s orders, if care of a dying patient is more than a family can manage.
