Care Management

Care management provides a professional, usually a social worker or a nurse, who evaluates an older person’s situation to determine what services are needed and to identify agencies that offer the appropriate services. The care manager sometimes also makes arrangements for the client to receive those services and checks regularly to be sure that the services are being reliably delivered. Some care managers work for seniors or their families, some work for service providers and primarily determine eligibility for service, and some represent insurance companies and must be concerned with keeping costs low.

Some social workers provide care management for a fee. Some care managers are certified as geriatric care managers by the National Association of Professional Geriatric Care Managers. Listings in this section are limited to agencies that have a certified geriatric care manager on staff, although there are also social workers who provide care management for a fee but who are not certified geriatric care managers.
