Case Management
Case, or care, management provides a professional, usually a social worker or a nurse, who evaluates an older person’s situation to determine what services are needed and to identify agencies that offer the appropriate services. The case manager sometimes also makes arrangements for the client to receive those services and checks regularly to be sure that the services are being reliably delivered. Some case managers work for an older person or his/her family, some work for a service provider and primarily determine eligibility for service, and some represent an insurance company and must be concerned with keeping costs as low as possible.
Listings in this section are limited to nonprofit organizations that provide free case management for senior citizens. Some social workers can provide case management on a private-pay basis. Some case managers are certified as geriatric care managers. See the "Action Guide."
Senior Citizens Information & Referral Service, 546-6262, can provide the names of certified geriatric care managers, both locally and nationally, who are registered with the National Association of Professional Geriatric Care Managers; call also for names of social workers who provide case management for a fee but who are not certified geriatric care managers (see page 209 for web site address).

In-home assessment, case management, and referral to services. Volunteers provide escorted transportation for errands, grocery shopping, and more. Volunteers make limited minor home repairs and safety modifications for older adults who meet eligibility requirements. No fees; contributions accepted.
